Registration

Registration Fee:   $100.00 (non-refundable) Per Child .   
Re-Registration fee is $150 after March 15, 2025. 

Student Technology and Consumable Fee (STaC) $200 per family (non-refundable) due with registration fee.

A Complete New Student Registration Packet Includes: 

  • Registration Paperwork filled out completely (located below) 

  • Registration fees

  • HSG Family commitment form completed

  • Catholic Tuition Grant Program (CTGP) Form completed  (For parishioners of parishes other than QH)

  • Birth Certificate (email copy to admissions @qofhschool.org)

  • Immunization Record (email copy to admissions @qofhschool.org)

  • Transcript Request Form (For students transferring in from another school)

Families looking to register for the next school year  should fill out the following forms. You can type directly on these forms, click the “download with my changes” (top right next to the print button), and then email the form to admissions@qofhschool.org
