Athletics Handbook & Registration

All students must meet all requirements below before they are permitted to participate in any school sport, including practices and games.

Participation Requirements

Each student is required to:

1. Submit a completed and current Permission Slip and Code of Ethics

2. Pay all applicable Registration Fees and Uniform Deposits

3. Have a valid Sports Medical Physical Form on file with the School Nurse

Students may not participate until all requirements are met.

Forms, Fees, and Deposits

For each sport and each season, families must submit the following:

• Completed Permission Slip

• $25 Registration Fee (per student, per sport)

• $75 Uniform Deposit (per student, per sport)

All materials should be sent to the school office in an envelope clearly labeled “Sports Forms.”

⚠️ Important Notes

• Do not give forms, cash, or checks directly to coaches

• No cash is accepted

• Students are not guaranteed a roster spot until all paperwork and payments are received

Submission Deadlines

All Permission Slips, fees, and deposits must be submitted by the following dates:

• Fall Sports: July 15
  (Please submit before the end of the school year if possible)

• Winter Sports: October 15
  (Winter season begins November 1)

• Spring Sports: February 15
  (Spring season begins March 1)

• A separate Permission Slip is required for each student, each sport, and each season

• The current form can be found under the “Handbook and Forms” tab on the school website

Registration Fees & Uniform Deposits

• Registration Fee:

  • $25 check payable to Queen of Heaven School – Sports Program

  • Please include on the check:

    • Student’s name

    • Grade

    • Sport

    • “Sports Registration Fee”

• Uniform Deposit:

  • Separate $75 check payable to Queen of Heaven School – Sports Program

  • Please include:

    • Student’s name

    • Grade

    • Sport

    • “Uniform Deposit”

  • The deposit is refundable and will only be cashed if the uniform is not returned

Sports Medical Physical Form

• A valid Sports Medical Physical Form must be on file with the School Nurse

• Forms are valid for one year

• If the form is valid on the first day of practice, it is valid for the entire season

• Students with expired or missing forms will not be permitted to participate

• Families will be notified if an updated form is required

Uniform Deposit Policy

Queen of Heaven’s Athletics Program follows a uniform reuse policy to help keep sports fees affordable.

• Uniforms are reused from year to year

• The $75 uniform deposit helps offset the cost of uniforms that are not returned

• Deposits are refunded at the end of the season when the uniform is returned

• A student will not receive a uniform for a future season until all prior uniforms are returned

• The deposit amount increased beginning in the 2022–23 school year to reflect actual replacement costs

Similar policies are used successfully at other Catholic schools.

Sports Sweatshirts are a Navy 1/4-zip pullover. The QOH Sports logo will be printed on the left front breast pocket, and the student’s name and jersey number printed on the back.  The left sleeve can be printed with “Basketball” or “Track” or “Softball” or “Soccer” or “Cross Country” or “Baseball” or “Volleyball” or “Hockey,” or it can be left blank. 

*** Parents and Coaches may order sweatshirts for themselves as well.  Parents/Coaches can have the QOH logo embroidered on the front instead of name/number/sport printed on the back and sleeve.
